  1. In a medium saucepan, combine milk, cream, and half of the sugar. If using a vanilla bean, split it lengthwise and scrape the seeds into the milk mixture; add the pod as well. Heat over medium heat until steaming, but not boiling.
  2. In a separate bowl, whisk together egg yolks, remaining sugar, and salt until pale and thick.
  3. Slowly pour about 1/3 of the hot milk mixture into the yolks while whisking constantly to temper them. Then pour the yolk mixture back into the saucepan with the remaining milk mixture.
  4. Cook over medium-low heat, stirring constantly with a wooden spoon or spatula, until the custard thickens enough to coat the back of the spoon (about 5-7 minutes). Do not let it boil.
  5. Remove from heat. If using vanilla extract, stir it in now. Strain the custard through a fine-mesh sieve into a clean bowl. Discard the vanilla pod.
  6. Let the custard cool to room temperature, then cover and refrigerate until completely chilled, at least 4 hours or overnight.
  7. Churn the chilled custard in an ice cream maker according to the manufacturer's instructions until it reaches a soft-serve consistency (usually 15-20 minutes).
  8. Serve immediately in waffle cones, or transfer to a piping bag with a star tip to pipe into cones for a classic look.
